Sam doesn't think things are ok, he's not stupid. He didn't think they were ok after Asylum either, but just like then, Dean is going to brush it off.
What Sam said was inexcusable to the point of me feeling sick. However, there is still some truth in what he is feeling. Dean is running from the demons, the angels, the war - everything - and Sam just wants to fight. He is frustrated and alone. He never asked for Dean to go in the first place, in fact, he would have never let Dean make that deal if he was still around. What he said crossed the line, for sure, and it was SO COLD, god - but remember - what Dean said was true, too. This Sam isn't the Sam we knew anymore.
